SHBG: What This Lab Marker Means in Testosterone Therapy

What Is SHBG?

SHBG stands for sex hormone-binding globulin, a protein made mostly in the liver that attaches to testosterone and other sex hormones as they travel through the bloodstream. When testosterone is bound to SHBG, it is not immediately available for the body's tissues to use. Testosterone that is not bound to SHBG or other proteins is often described as "free" testosterone, which is the portion many clinicians consider more directly active. Because SHBG levels vary from person to person based on factors like age, body composition, thyroid activity, and general health, the same total testosterone number can mean different things for different people depending on how much of it is actually bound up by SHBG.

Why SHBG May Come Up During a Testosterone Evaluation

If you're asking questions to discuss low testosterone or preparing for a consultation, SHBG is one of the values a clinician may want to review alongside total testosterone. A person with high SHBG could have a total testosterone number that looks adequate on paper, while their free or usable testosterone is comparatively low. The reverse is also possible. Reviewing SHBG together with total testosterone, free testosterone, and symptoms is one way a clinician builds a fuller picture during an evaluation, rather than relying on a single lab value in isolation. This is part of why lab review conversations tend to involve more than one number.

What This Might Mean for Your Conversation

You don't need to interpret your own SHBG result before a consultation, but knowing the term can help you ask better questions. If you've had bloodwork done, bringing those results to a consultation gives a clinician more context to work with. If you haven't had labs done yet, that's a normal starting point too. Whether SHBG testing is included in your specific evaluation depends on individual clinical review, since testing needs vary by situation.

Where SHBG Fits in the Testosterone Therapy Program

SHBG is most relevant to the evaluation and ongoing lab-review side of a testosterone therapy conversation. If you want a closer look at how lab review generally fits into a testosterone consultation, the Evaluation & Lab Review page walks through what that discussion may involve. You can also browse the Glossary for other terms that often come up alongside SHBG, such as total and free testosterone.

FAQs

Is a high SHBG level a problem?

Not necessarily on its own. SHBG naturally varies between individuals, and what matters is how it's interpreted alongside total testosterone, free testosterone, and your symptoms. A clinician can discuss what your specific numbers may suggest during a consultation.

Can SHBG change over time?

Yes, SHBG can shift due to factors like aging, changes in body composition, thyroid function, and other health conditions. This is one reason ongoing conversations about lab values, rather than a single one-time check, are common in hormone-related care.

Do I need to know my SHBG level before requesting a consultation?

No. You're welcome to request a consultation whether or not you've had any labs done. A clinician can discuss what testing, if any, makes sense for your situation.

Is SHBG the same as free testosterone?

No, they're related but different. SHBG is a protein that binds testosterone, while free testosterone refers to the portion of testosterone that isn't bound and is available for the body to use. Both may be discussed together during an evaluation.

Why do some lab panels include SHBG and others don't?

Panel content can vary depending on what a clinician determines is relevant to your individual evaluation. Whether SHBG testing applies to you is something to raise during a consultation rather than something the same for every reader.

What questions should I ask about SHBG during a consultation?

You might ask how SHBG relates to your total testosterone result, whether your levels seem typical for your situation, and what, if anything, a clinician would want to monitor over time.
